Age Requirements
Florida law states that a child must be six years old on or before September 1st in order to enter first grade, and the child must have successfully completed a kindergarten program. The age requirement is five years old by September 1st for kindergarten entry. A birth certificate is required of all pupils new to Orange County.
Immunizations
State law and school board policies require evidence of immunizations for the following communicable diseases:
DPT Diphtheria, Pertussis, Tetanus 5 doses
Polio 4 doses
MMR Measles. Mumps, Rubella 2 doses
Hepatitis B (K, 1st, 2nd and 3rd) 3 doses
Varicella (chicken pox) PreK and K 2 doses
This evidence is required of all first-time students entering PreK, kindergarten or first grade and all new students from out of state. Please comply with the above regulations and obtain form HRS 680 from your doctor or Public Health Clinic. The East Side Health Clinic is located at 12050 E. Colonial Drive (407 249-6232). If immunizations are complete, HRS 680 can be issued during the visit. The HRS 680 (Certificate of Immunization) will become a part of your child's permanent school health record.
Health Records
All students entering Florida schools for the first time need an immunization certificate (HRS 680) and a physical examination completed within the last year.
Proof of Residence
Proof of residence may be provided in the form of a current homestead exemption certificate, a lease agreement, a notarized manifestation of domicile or a notarized statement of residence.
